Flu vaccine requirement due Sunday, Dec. 1
As a health system, we are committed to the health and safety of our team members, volunteers, patients, residents, members and visitors. In that spirit, the influenza or flu vaccine remains a requirement for all employed and non-employed team members including medical staff, volunteers and students who may work on-site at any time (which includes roles in the on-site and hybrid workplace category), while strongly encouraging the vaccine for those who are fully remote with no on-site responsibilities for their role. Please reach out to your leader to determine if the flu vaccine requirement applies to you.
Vaccines will be offered at our on-site clinics and team members must receive their vaccine by Sunday, Dec. 1. Failure to comply with the requirement — having an approved exemption or being vaccinated — will result in voluntary resignation.
Please complete the following steps prior to your vaccine visit:
- Log into ReadySet to complete your Flu survey; this is required prior to your vaccine visit.
- If you are under 18, please print this consent form. The consent form must be signed by you and your parent/legal guardian. Please bring the signed document with you to an EHS vaccine clinic.
- Visit a walk-in on-site clinic. No appointments are needed.
- After receiving a vaccine team members must verify vaccine documentation in ReadySet.
- Non-employed Independent Providers – please retain a copy of your personal proof of vaccination. Flu documentation does not need to be sent to Employee Health Services. Please contact Medical Staff Leadership with questions.

You may obtain a flu vaccine from another health care facility, such as your primary care physician or a local pharmacy. Please upload your documentation to your ReadySet account by following these instructions. All records must be sent by the Sunday, Dec. 1 deadline.
Please note: Flu shots received outside of EHS are not reimbursable. However, they are covered 100% by most group health insurance plans, including Priority Health. Check with your health insurance carrier for coverage availability.
