An expansion of the current PCMH Program is designed for SCP who are considered the “neighbors” with whom PCPs work to deliver the best care possible to patients. BCBSMs vision is for the PCMH-N program, along with the PCMH program, to help create high functioning health care “neighborhoods” and overall Organized Systems of Care.

Beaumont ACO’s goal when building PCMH-N practice is to engage specialists in building processes within their office that will support population management, ensure effective communication and coordination of care with PCPs, provide appropriate and timely consultations and referrals and improve the care and outcomes of our shared population.

Beaumont ACO program coordinators will be reaching out to specialist offices to begin work on transforming your practices into a PCMH-N practice. PCMH-N participation is a requirement of your PGIP engagement with Beaumont ACO.
