To better ensure the health and well-being of the Beaumont team, patients and families, a flu vaccination is required for all employed physicians, and those with active hospital clinical privileges and their advanced practice professionals/providers – unless a medical or religious contraindication exists.

Failure to obtain a flu vaccine or submit the declination form by Dec. 15, will result in loss of access to patient charts within Epic/oneChart.

Vaccination clinics are scheduled across Beaumont at various locations. Calendars with flu clinic locations, dates, times and consent/declination forms are posted. If receiving a flu vaccine at a hospital, please complete an online consent form first. Physicians will need to have their Doctor ID and NPI numbers available.

Medical staff members may obtain a vaccination anywhere, but they must provide all required documentation. Providers can easily access and submit their offsite flu vaccine information without using the paper flu form by using this link.
