Transitions of Care/ Provider Delivered Care Management (PDCM)

The Beaumont ACO recently launched the use of Patient Ping, a national care coordination network that connects physicians with real-time health care events of their patients. Each Beaumont ACO practice participating in the PGIP program has been granted access to Patient Ping to allow for tracking and care coordination of patient admittance, discharge, transfers all hospital systems in the state of Michigan, including some skilled nursing facilities. Practices are now able to receive real-time alerts via email or text on patient ADTs, allowing for an increase in post discharge transitions of care visits within seven or 14 business days.

In addition to Patient Ping, Beaumont ACO has launched centralized care management services for BCBSM PPO & MA PPO populations. In 2018, BCBSM launched the expansion of Provider-Delivered Care Management benefits for its members allowing for more care management services to be provided that can improve patient care. This expansion pushed for Beaumont ACO to launch PDCM nurse care management services that will both improve patient outcomes and place our physicians in the position to receive additional value-based reimbursements.
